Lesion A or B? 👇 Answer below This case required all three views: clinical, dermoscopic, and microscopic. Clinically, both lesions appeared pearly and annular, with subtle pigmentation and visible vessels. Dermoscopy brought us closer but also demonstrated just how much basal cell carcinoma and trichoepithelioma can overlap. Both showed pigmentation without a melanocytic pattern and branching vessels. Lesion A had more prominent, sharply focused arborizing vessels, while lesion B had finer vessels and features suggesting follicular differentiation. The answer 👇 Lesion A is a basal cell carcinoma, a form of skin cancer. Lesion B is a trichoepithelioma, a benign hair follicle tumor. Under the microscope, the basal cell carcinoma showed basaloid tumor islands with peripheral palisading and retraction clefts. The trichoepithelioma showed follicular differentiation with horn cysts and papillary mesenchymal bodies. Sometimes, the challenge continues under the microscope. Morpheaform basal cell carcinoma can closely resemble desmoplastic trichoepithelioma, sometimes prompting the use of immunohistochemical stains as an additional diagnostic tool.
