He saw him walk and based on his experience he can assess the difference between a cramp and an actual injury limp. The player just wanted an excuse to delay the game and rest (which is not allowed).

What people don’t realize is that rules don’t allow medical timeout for cramping. That’s why the player says it’s pain and not cramping because he is buying time. He also told medical to “stop acting stupid!”
