Mounjaro, Wegovy and Ozempic: Persistent Vomiting, Thiamine Deficiency, Wernicke Encephalopathy and When to Contact NHS 111 🧠 IF YOU’RE TAKING MOUNJARO, WEGOVY OR OZEMPIC AND YOU’VE BEEN VOMITING FOR DAYS, PLEASE DON’T ASSUME IT’S “JUST A NORMAL SIDE EFFECT”. Here’s the question I’d ask you: ARE YOU STILL MANAGING TO EAT PROPERLY AND KEEP FLUIDS DOWN? Nausea and vomiting are recognised side effects of these medicines. For many people they are mild or short-lived. But repeated vomiting, a major loss of appetite, rapid weight loss or barely eating can create another problem: NUTRITIONAL DEFICIENCY. A 2026 pharmacovigilance study identified 15 cases of Wernicke encephalopathy in people using GLP-1–based medicines. Eight involved semaglutide — the active ingredient in Wegovy and Ozempic. Six involved tirzepatide — Mounjaro. So 14 of the 15 cases involved one of those two medicines. And 13 of the 15 people had gastrointestinal or nutritional problems such as vomiting, major weight loss, loss of appetite or malnutrition. Wernicke encephalopathy is a rare but serious neurological condition caused by severe deficiency of vitamin B1 — THIAMINE. Your brain needs thiamine to use energy properly. Warning signs can include: • NEW confusion • difficulty walking or poor balance • unusual or abnormal eye movements And here is an important detail: You do NOT have to have all three. In this study, only 2 of the 15 cases had the full classic triad. Seven of the 11 patients with follow-up information had long-term neurological problems. But please keep this in proportion. Fifteen cases do NOT tell us how common this problem is. There is no reliable figure from this study for the chance of Wernicke encephalopathy in an individual patient. Thirteen cases came from the FDA adverse-event reporting system, one from published literature and one from the researchers’ medical centre. These reports can identify a SIGNAL, but they cannot prove that semaglutide or tirzepatide directly caused every case or calculate a true incidence rate. The likely concern is what can happen when severe vomiting, very low food intake or rapid weight loss leaves the body short of thiamine. So if you’re taking one of these medicines and vomiting repeatedly, ask: Can I keep fluids down? Am I eating enough? Am I losing weight extremely quickly? Am I becoming weak, dizzy or dehydrated? If you keep being sick and cannot keep fluids down, NHS advice is to contact NHS 111. And if prolonged vomiting or poor nutrition is followed by NEW CONFUSION, major difficulty walking or balancing, unusual eye movements, severe drowsiness or another sudden neurological change, that needs urgent medical assessment — don’t wait for every symptom to appear. Also seek urgent help for severe, persistent abdominal pain that may travel to your back, with or without vomiting, because GLP-1 medicines can rarely cause acute pancreatitis. Please don’t stop Mounjaro, Wegovy or Ozempic simply because you’ve seen a frightening headline. But persistent vomiting is NOT something I want you normalising just because nausea and vomiting appear in the leaflet. Contact your prescriber.
