@sam.holvey: Have you ever seen a facility fee on your medical bill or Explanation of Benefits and wondered why you’re paying for the “facility” on top of the doctor’s visit? There’s a reason for that, and it has a lot to do with hospitals buying physician practices and the way American healthcare pays differently depending on who owns the building. In this episode of How American Healthcare Got This Way, I’m breaking down how we went from independent doctor’s offices to massive hospital systems, why the exact same visit can cost more after an acquisition, and what patients should know about facility fees. #Healthcare #MedicalBills #FacilityFees #HealthInsurance #PatientAdvocacy

Would also argue that older physicians sold their practices to hospitals, cashing out and having millions to retire with. Meanwhile, they left younger physicians screwed. Boomer physicians got the best conditions to practice. Started with higher salaries, paid more by insurance companies, and then when they reached retirement, they sold everything and left junior doctors to suffer under this hospital based indentured servitude.

Things going to get even more interesting when you will only be seeing PA for all subsequent visits. Healthcare quality is going down while insurance premiums are going up every year.

It's way too complicated to run your own practice if you see Medicaid or Medicare patients. The government has made it overly complex and there are penalties for not meeting " quality/best practice" guidelines. They want cookbook medicine so Healthcare can be delivered by non-physicians. Too many headaches. Too much financial risk.

Private practice for 30 years in general surgery. My last year in private practice I took home $300,000 before taxes. There were weeks that I did not take a paycheck to pay my overhead. Joined a hospital. RVU based. Took home $700,000 last year working much less than before. Not spending weekends doing the books, payroll etc. I had no choice.

I just recently retired as an MD and I sold my practice to a larger entity. It’s getting harder and harder to survive unless you’re in a niche specialty. The hospital chains are buying up the primary care physicians, who then refer patients to doctors who work for the hospital chains. The pie is getting smaller and smaller for independent people. They say there’s a doctor shortage, but they’re really isn’t a doctor shortage. If there was, you could open up your own gig and have a go at it but it’s not that easy.

I worked for a large, hospital owned office with 8 physicians who all came from their own private offices and it was a nightmare watching the corporate consultants take all the soul of out medicine.

Instead of Federal single payer universal healthcare our political leaders decided to give for profit insurance companies management of our healthcare. And doctors said nothing. Now doctor work for insurance companies and insurance companies own our politicians.😂

Life was much better when the doctor owned the business. Now rich greedy people own the business and are not bashful about sticking an IV into your bank account!

Here the Doctors have left working at hospitals and formed a 6-10 MD practice with NP’s because they were tired of being controlled, and even bought their own surgical center.

We recently had a huge facility fee attached to a visit at an out patient office visit. Even with two good health insurance polices, we still had to pay several hundred dollars out of pocket. After many emails and calls, we finally got the explanation. This was for a basic visit where normal lab results were reviewed. Insane!!
