A “normal” FSH does not automatically mean your ovarian reserve is fine. This is exactly why I don’t interpret fertility blood tests one marker at a time. FSH is produced by the pituitary gland and helps stimulate follicle development early in the menstrual cycle. As ovarian reserve declines, the ovaries can become less responsive to that stimulation, which can result in the pituitary producing more FSH. But here’s where interpretation gets more interesting. If your oestradiol is already elevated on day 2 or 3, that oestradiol can exert negative feedback on the pituitary and suppress FSH. So you can potentially have an FSH result sitting comfortably within the reference range while the oestradiol result beside it changes how I interpret that number. This is why I want context. I want to know your age, cycle history, AMH, day 2 or 3 FSH alongside oestradiol, and ideally your antral follicle count where appropriate. And even then, ovarian reserve and egg quality are not the same thing. This is the level of detail we go into when assessing fertility at Nourishing Apothecary.
