As the $239 million spacecraft was being rotated, it tilted just 13 degrees before falling roughly three feet onto the floor. Investigators discovered the unbelievable cause: 24 bolts that should have secured the satellite to its turnover cart had never been installed. The damage ultimately cost the U.S. government roughly $135 million to repair.
