As an ER RN, I always like to throw this little breakdown out there to explain staffing costs. My ER probably has an average pay of $40/hr. We have a 4:1 patient ratio. So each patient pays $10/hr for the nurse’s time. Seems reasonable…comes in, gives some pills, takes your blood pressure, etc. But then what about your neighbor, the ICU patient? I’m the only thing standing between them and dying. If I mess up their titration or don’t communicate a status change to the doctor, serious stuff can happen, including death. Ok, let’s talk about that part. You died and EMS brought you in coding. Let’s say we have four RNs in the code for an hour. That’s fairly reasonable. That comes out to $160 in staffing during that to bring you back to life (hopefully). Seems very reasonable! A bargain! Who *wouldn’t* drop a couple hundred on that? Except we have fifteen other patients splitting that cost with you! As a comparison, a saline flush that probably costs $.25 costs $6 at my facility. If I am giving you multiple IV medications a couple of times over your visit, I very likely actually paid for myself just in flushes used. So when you see staffing fees, etc., remember that’s what we really get paid and how that actually works. They may charge you $300 in staffing, but I don’t ever see it. If you’re there for four hours, I saw $40 of that. Other people who you never met are getting paychecks (significantly larger than mine) from those bills.

My friend you need to stockpile some suture material and lido for next time and have a friend do it, then go to urgent care and get the tetanus shot for way less than that (but I understand I got a needle stick on a rotation and they forced me to go to their ER and wouldn’t give me the choice to not and it cost me $700 for labs after insurance. I was pissed they wouldn’t just check the guy for hep b and hiv and if it came up neg let me not go)

I work as an ER nurse. I had just gotten off of shift and maybe an hour or so afterwards, I suddenly lost vision in my right eye. I drove my butt back (on one eye like a crazy person), and of course they thought I was having a stroke 😳 anywhoooo, I very well could’ve been having one, buttt I still refused tpa. Butttttt just my ER bill was 21K. But nonetheless, very thankful and grateful (to see the same doc I JUST got done working with 🤣) jk he knows I appreciate him very much

I got like 3 different bills when I went. All wrong. The borderline vitriolic phone calls I had to make for them to properly run my insurance and send a proper, final bill. Made me appreciate more why my patients are always so frustrated with the experience. Also made me question why I even bothered trying to do it properly when I could’ve just gotten it done by a colleague the next day at my clinic instead 🤦‍♂️
