got a weird one... male 50s with history of pack a day smoking, obesity and family history of heart. sudden onset of ripping/tearing chest pain (they could NOT find a better or different word) which referred into the back at the same level, diaphoretic, dizzy with a pre-syncopal event at onset. 12 lead normal, pressure at approx 100-110 systolic and they state their L arm feels "clotted" (again literally no other descriptive type words they could find). we bring them to the ER and come back couple hours later and all tests including chest CT and blood work show literally nothing. repeated 3 times. thoughts?
