A new systematic review and meta-analysis in The Lancet pooled 1,904,237 total hip arthroplasties from 80 joint registries and 29 clinical studies, examining only contemporary bearings: highly cross-linked polyethylene against metal or modern ceramic heads, plus third and fourth generation ceramic-on-ceramic. Registry survivorship was 93.6% at 20 yrs, with extrapolated survivorship of 92.8% at 25 yrs and 92.1% at 30 yrs. For context, the 2019 analysis most of us have quoted for a decade reported roughly 58% survivorship at 25 yrs. That work was rigorous but also built on prostheses that are largely no longer implanted. Why the numbers keep improving: Aseptic loosening has driven close to half of all hip revisions, and it is fundamentally a wear problem. Debris from the polyethylene liner can provoke an inflammatory cascade, osteolysis follows, and fixation fails. Highly cross-linked polyethylene reduced that wear rate substantially, and registry data now show the share of revisions attributed to aseptic loosening falling over time. Modern ceramics have addressed most of the fracture risk that limited earlier generations. The idea that a hip replacement buys you 10-15 yrs describes technology from a different era.
