@dr.kavita.t: Replying to @Magi Findlay we need to always understand the principles behind an intervention. So top up absolutely be necessary but why. Either not enough milk being made (glandular problem) or baby is not effectively creating supply (mechanical issue assuming baby has not underlying health issues - which is actually rare!) So for either of these reasons we need a little more than just mindlessly trying to ‘empty’ a breast. It just work with the breasts, chemicals, hormones etc. Now, please ALWAYS follow your own teams advice! I am only ever giving general Information! But understand what’s happening. Thank you. We ended up adding a bottle of formula a day which wasn’t as scary as I thought it was going to be. That plus a few EBM bottles a day and his weight is piling on!

As someone who triple fed due to poor weight gain with no transition plan for months and months and months. Thank you. This actually makes sense. I wish more professionals had and shared this knowledge. I now understand the schedule I was on had no hope in heck of protecting or improving my supply (it was just torturous - a feed immediately followed by a pump and bottle every 3 hours). The order and timing of pumping in hindsight was completely wrong and baby’s allergies totally dismissed for way too long as a huge part of that picture despite parent advocacy. Thank you for all that you do.

my son was born and weight 3.290kg...he didnt come back to his birth weight until he was 3 weeks( i had issues with feeding) but I believe it's under control now. he is now 6 weeks old and this past week he gained 170grams. my gp still wants me to top him up with breastmilk after feeds. what is your opinion on this? I thought as long as he is gaining weight within that 150-200 gram range its fine. for reference he is now 3.955kg

What about if they have adequate nappies. Supply appears sufficient. Why would top ups be needed and weight gain not occur?

I had twins at 35 weeks. The first 10 days I breastfed, fed baby formula to prevent weight loss, and then pumped. Since 10 days I’ve been producing enough in pumping to exclusively feed the babies my breastmilk. They are 9 weeks old now (4 weeks adjusted)and have doubled their birth weights (both over 10lbs now). I want to transition them to nursing from the breast so that I don’t have to pump so very much and have kind of been getting discouraged because even if I do it for one feeding it feels like it isn’t going to end. They nurse and fall asleep and then wake up and nurse again and then are hungry again. Any suggestions to help make it seem less overwhelming? I have successfully breastfed my older four kids and am still also tandem feeding my 25 month old in addition to the twins at nap and bedtime, so I know how to do this I just get overwhelmed and anxious that they aren’t getting enough that I give up and keep pumping and bottle feeding.

Have been triple feeding my 2.5 week old since 1 week she can latch to the shield on right side (other side is inverted so not trying that side as much for bf) and pumping 7 x daily getting between 700-800mle now and she takes about 60-80ml per bottle is that a decent supply to look at doing the 12 hr apart bf attempts? She has just gone 200g over her birthweight yesterday

Can you speak about postpartum thyroiditis and low milk supply? I’ve tried everything to boost my supply. Currently top up with formula 6 x a day 120ml (low weight gain early on) he is 11 weeks. I’m hyperthyroid right now, I believe it’s causing a slow let down. Can only pump max 30ml (even away from feeds).

You may have already answered this but do you have a primary pump recommendation for 7-8 pumps per day with the option of wearable cups when out and about? Replacing my medala swing maxi and looking for suggestions as it essentially will be my new best friend 🥲😂
