Clownfish in a sea anemone — watch these colorful reef fish swim and dance through the protective tentacles of their underwater home. 🐠🪸✨ Their fascinating relationship is one of the most recognizable sights on a tropical coral reef. Can you have PCOS if you’re not overweight? Absolutely. And this is something I wish more women understood. One of the biggest misconceptions about polycystic ovary syndrome (PCOS) is that it only affects women who are overweight or struggling to lose weight. But PCOS is not a weight disorder. It’s a complex reproductive and metabolic condition involving interactions between the ovaries, brain, hormones and metabolic signalling. And women can experience PCOS at any body size. You might have heard the term “lean PCOS”. This isn’t a separate medical diagnosis or a completely different type of PCOS. It’s simply an informal term used to describe women with PCOS who have a BMI within the normal or lower range. The diagnostic criteria are still the same. But here’s where things become particularly interesting. You can be lean and still have insulin resistance. Insulin resistance occurs when tissues such as muscle, liver and fat become less responsive to insulin. To compensate, the pancreas may produce additional insulin to maintain normal blood glucose levels. This means someone can have normal fasting glucose while their body is producing more insulin than expected. And insulin doesn’t just regulate blood glucose. Higher insulin levels can stimulate ovarian theca cells to produce more androgens, including testosterone. Insulin can also reduce the liver’s production of sex hormone binding globulin (SHBG). SHBG helps bind circulating sex hormones, including testosterone. When SHBG decreases, the proportion of testosterone circulating in its unbound, biologically active form can increase. For some women with PCOS, these hormonal changes contribute to acne, excess facial or body hair and irregular ovulation. But there’s another important distinction. Not every woman with PCOS has insulin resistance. Some women with lean PCOS have relatively normal metabolic markers but still experience androgen excess, irregular menstrual cycles or difficulty conceiving. PCOS involves more than insulin alone. The hypothalamus and pituitary gland influence ovarian hormone production through LH and FSH signalling. The ovaries produce androgens and develop follicles. The adrenal glands can also contribute to circulating androgens. And metabolic factors can interact with these reproductive pathways. This is why I don’t believe every woman with PCOS should automatically be given the same treatment plan. When I’m assessing someone with PCOS, I’m interested in understanding their individual presentation. Are they ovulating regularly? Are their androgens elevated? What does their SHBG look like? Is there evidence of insulin resistance or impaired glucose regulation? What does their lipid profile show? Have other conditions that can cause similar symptoms, such as thyroid dysfunction or elevated prolactin, been excluded? And importantly, what are their goals? Someone wanting to regulate their menstrual cycle may need a different approach from someone preparing for pregnancy or managing metabolic risk. Testing also needs to be individualised. Fasting glucose, HbA1c and sometimes an oral glucose tolerance test can help assess glucose regulation. Insulin measurements may provide additional context in selected cases, but they are not recommended as routine diagnostic tests for PCOS because of their limitations. A normal BMI doesn’t rule out PCOS. A normal fasting glucose doesn’t necessarily rule out insulin resistance. And having PCOS doesn’t automatically mean you need to lose weight. The goal is to understand the hormonal and metabolic factors relevant to that individual and develop an appropriate management strategy.
