I have bipolar II but it was so hard to diagnose because my hypomania was hard to identify. I have fibromyalgia so I’m always tired. I also have ADHD so I’m always wired. I say I’m always tired and wired 😂 so my hypomania, I thought was just a few good days, but it was actually hypomania.

Thank you for this, Doctor. As a psychologist, I’m seeing this far more often than I’m comfortable with. People come into my office with an existing Bipolar I or Bipolar II diagnosis, and when we actually explore their history of mania or hypomania, the episode simply isn’t there.
I’ll hear, “I didn’t sleep for days,” but when I ask further, they were exhausted and desperately wanted to sleep. Or, “I spent a ridiculous amount of money,” but the same impulsive spending also happens when they’re depressed. Then we start looking for the rest of the syndrome: Where was the increased energy? The distinct change from baseline? The inflated self-esteem or grandiosity? The other symptoms occurring together during a discrete episode?
I’ll use the MDQ as part of the screening process, but obviously a screener isn’t a diagnosis. Once I actually explain what mania and hypomania look like clinically, I have watched people experience enormous relief realizing that what they’ve been experiencing may not be bipolar disorder at all.
We have got to stop diagnosing bipolar disorder from isolated symptoms. Not sleeping isn’t automatically decreased need for sleep. Impulsive spending isn’t automatically mania. The fucking episode matters.
Thank you for talking about this. It needs to be discussed far more often.

My son got diagnosed at 5 years old with bipolar. He is now 20 years old, and even on his meds was making bad decisions. He moved out out of our house and stopped taking all of his meds, and he says that he feels better. I don’t know if we meet the wrong call on having him start meds when he was that young. It was such a tough time and we trust in love our Doctor but now I am wondering is he actually bipolar or did they make a mistake?

Hypomania is incredibly challenging in general because everything is a spectrum, not a yes/no. Everyone has good days and bad days, good weeks and bad weeks. Especially people with ADHD and AuDHD. We are simultaneously tasked with not pathologizing normal emotional variability while also recognizing psychiatric pathology (early) based on a 15-45 minute appointment and collateral info from other flawed people. It’s very frustrating.

I feel like my ex fiancée has been manic for 9 months ever since she started working the night shift from 10 pm- 6am. She takes only 75mg of lamotrogine and misses doses. Since then she cheated with a co worker we had broke up sold out condo and are currently fighting for custody of our 10year old autistic son . She lives in a basement now and I had to move back with my mother . Can mania last this long if untreated and night shift sleep deprived? Anyone please let me know

Is it possible to only have a manic or hypomanic episode as a result of a medication and not have bipolar disorder? I had 2 episodes in my life and both only occurred directly after initiating psychiatric medications. I never had any symptoms prior. I think that’s why they were caught so quickly because they were so drastic from my baseline. Prozac induced a mixed episode characterized by mania, depression, and psychosis. Intuniv induced a hypomanic episode followed by psychotic depression after discontinuation. I am autistic and have many adverse and paradoxical reactions to medications, so I’m not sure if that had something to do with this? Also, I did want to note that when it comes to the label of “antipsychotics,” I stopped calling them that because of the connotation. Instead, I call them dopamine and serotonin antagonists/modulators. I don’t understand why we don’t do this normally given that we have SSRIs and SNRIs being classified based on their mechanism of action.

It was always misdiagnosed w depression because my hypomania was so “positive”. I took 18 hours of college classes, worked full time, ran 5-6 miles a day, party every weekend for days in a row. But then I’d crash, want to ✨ go get help, diagnosed with depression, rinse, repeat.

I finally refused to keep taking lithium due to side effects and I have severe bipolar 2 depression issues. I’m starting lamictal and I’m hopeful because it’s about the last thing I can try atp 😭 these comments are giving me hope

Yes my client ended up in Vegas. Drove from Detroit over one day and one night. Was found naked in an alley in a Bentley he had purchased the night before- spent his entire retirement. He was kicked out of the casino that evening. The rest is fill in the blank. Crazy story and very sad.

Also worth noting that depressive morbidity predominates over mania/hypomania in the disease course of both subtypes! Most people with bipolar will mainly be depressed as opposed to manic, which is feel is a huge frame shift that needs to happen when approaching the diagnosis.
