Former ICU nurse here. Intubation, feeding tubes, antibiotics. Those were the cornerstone of the unit, and almost every shift meant a family conversation about them long after the diagnosis was terminal and the patient could no longer speak for themselves. It's always their choice, but in the ICU it rarely felt that way. It's part of why I moved to inpatient hospice. Thank you for this.

Cancer survivor here. I’ve actually thought very seriously about all of this. You make very good points. However, my goal is if I’m diagnosed with a terminal disease the first thing I will do is go to the store and buy the most luxurious set of sheets that I can find. In fact, I’ll buy two sets. I know I’m gonna be spending a lot of time in bed so I want it to be comfortable and luxurious. After that, I would go to the store and buy all the food that I’ve been denied myself over the years because I don’t wanna get fat. I would gorge on that. The next day I would do all the things that you mentioned.

We had my husbands celebration of life while he was alive, outdoors, fave foods, fam & friends. He was so delighted it was like a bday party. He died 8 months later

What if the terminal patient cannot emotionally deal with her disease advancing. She just cries when she thinks her tumor is progressing and cannot face making any of these decisions. Just meet her where she is I suppose. Very hard.

I would also appoint a legal medical decision maker that can be trusted to abide and protect by my wishes. Too often I’ve seen family go against the patient’s wishes once the patient loses capacity.

Speaking to your family members about your advanced directives and how you want them followed no matter what is soooooo important. ICU RN, it takes so much burden off family members 💙

This is great advice! One thing I would add is that I would try to do all the things I always wanted to do but never had the time- like in the Bucket List. I think it would be a lot of things we take for granted as we get busy with like watching a sunrise or sunset, take a trip to a secluded beach, use the fine china, order room service… lol just enjoy the time I had left before things get bad.

As someone who has just lost two parents 8 months apart and having to deal with sooo much admin. Write down all my passwords for the person having to deal with all those things whilst I still had all my faculties. Transfer anything and everything I possibly can to the people meant to get them so they dont have to deal with the admin. Close everything that can be closed. Drink all the wine, especially the good stuff, its been in my cupboard for 10 years, it dosnt need to take up space in theirs for the next 10 😂

Australia here. My partner has been diagnosed with biphasal pleural mesothelioma. He had an Advanced Care Directive, but it’s been updated in view of his condition. We already had Enduring POA. He has been approved for VAD, which here means a life expectancy of less than 12 months. Our immediate family know and support his decision. The administrative work in all this has been a lot more than I expected. We live in a regional area and have met the two nurses, one of which is likely to administer his end of life preparation. Very lovely people. He and I disagree about his funeral but he’s coming around 🤣. Even our neighbour, who is a first responder, knows he is not to be resuscitated.

My mom had nothing in place when she was diagnosed with terminal lung cancer, we scrambled to do a holographic will and get a POA and directives done. About 4 or 5 days after she signed everything she had a brain bleed and lost use of her dominant hand and wouldn’t have been able to sign anything, so I’m glad we got it done when we did. Everything moved so quickly from her diagnosis and she was gone 48 days later.

My mom did that for us. She saved money for caregivers. It’s really helped. She’s going to run out but it’s still a huge help. 🥰 Gives us time to get organized.

I'm an only child and forever grateful that we always talked about death fairly openly. When Mom got cancer she looped me in to a meeting with her financial planner, and reminded me of where her calendar was and where I could find her login info for different things. I had known where those planners were well before she was sick, but it was nice to have reassurance. Her organization has absolutely made this process significantly easier on me and my dad. I would never expect someone who is not a Virgo that was in her 20s during the height of Martha Stewart to even think to do these things, but logins and phone or email password are so so so important.
