She says the first rule of structural design is “read the fucking syllabus.” I’d argue that an even more important rule of design is: make your instructions so clear and unambiguous that there is as little room for error as possible.
You don’t design a system on the assumption that everyone will interpret everything perfectly. You design it knowing that people make mistakes, misunderstand instructions and take shortcuts.
NASA learned that lesson rather spectacularly with the Mars Climate Orbiter. One team was working with imperial units while another expected metric units, and a spacecraft worth hundreds of millions of dollars was lost. The problem wasn’t simply that somebody should have “read the instructions better”; the system allowed a foreseeable misunderstanding to become catastrophic.
It reminds me of something a project portfolio manager once told me. During the Iraq War, while serving in the Australian Navy, an American ship contacted them asking whether they had seen a missing “biological mine detector.” The Australians said no. Only later did they realise the Americans meant the dolphins they had already seen.
His lesson was simple: if you mean dolphin, say dolphin.
Murphy’s Law applies to design too: if there are ten ways to do something correctly and one way to do it disastrously wrong, sooner or later somebody will find the eleventh way you never even thought of.
Good design doesn’t blame the user for misunderstanding. It tries to make misunderstanding difficult in the first place.

Survival in the real world requires ignoring obviously stupid instructions by people who are temporarily in charge of you. If she makes a single mistake on her syllabus, she’s teaching people to follow it no matter what.

The band Van Halen did this with their contracts for venues and places to play. In between the whole contract, Van Halen asked to have in the room, only certain colors of M&M’s. When they got to the venue and saw the M&M’s were not correct they knew they could cancel the whole venue because the person never read it or they didn’t read all of it.
