The 2022 Moncrieff umbrella review in Molecular Psychiatry confirmed what many of us have suspected for years: there’s no convincing evidence that depression is caused by reduced serotonin activity or concentrations. But here’s the thing — that doesn’t mean SSRIs are useless. It means we’ve been using a marketing framework to explain a real treatment effect. The current model is more nuanced: • Monoamine-glutamate continuum • Reduced BDNF and hippocampal volume • Impaired neuroplasticity • Ketamine’s rapid antidepressant effect is proof-of-concept for the neuroplasticity model A treatment can work without its proposed mechanism being correct. Aspirin. Lithium. ECT. Ketamine. What I tell patients now: ‘We don’t fully know how these work, but the evidence is that they do. Unfortunately many drs tell their patients they have a chemical imbalance and their body needs an antidepressant to correct that. I don’t agree with that. Drs are not being honest to patients

They shouldn’t be prescribed so easily in my opinion until the majority of providers that are prescribing them, are capable of ruling out Bipolar 2. I believe that is where a lot of the issues stem. They are quickly diagnosing anxiety, ocd, depression but can’t recognize hypomania or the long bouts of depression that almost feel like a bipolar 2 persons baseline. And then they give that SSRI or SNRI to that patient & it turns into a nightmare situation for the patient. Then they need to deal with both the mental and physical aspects of weaning from it when they are already in a mentally vulnerable state. It puts so many people on a never ending cycle 🔄

I think antidepressants work because they reduce or blunt negative emotions but for some over time than can blunt positive emotions
