As a psych graduate I would say that oddly the way he behaves (if it continued outside of a killing game context) would seem to stem from a complete lack of trust in other people associated with an anxiety disorder, CPTSD or paranoid personality disorder rather than a lack of empathy like he wants people to believe

I never thought of AsPD as a possible diagnosis for him or understood people who think so because kokichi's actual character is extremely different from how the fandom perceives him and his disruptive actions are performative not authentic

i personally agree because alot of his actions tie back to him coping with the fact hes in a killing game...the flashback with kaito made me think hes probably in control of his personality and mental wellbeing more than many think.

there's lots of context missing from the game too! (and the english translation doesn't make justice to a lot of his character, bc he's actually a lot more intentional about his lying in jp for example, and they made him much more impulsive and whatnot in the localization) so ofc all this makes a proper "diagnosis" much more difficult imo ... for one, it seems like he's prone to dissociation (in a qna, kodaka was essentially asked "how did kokichi manage to get through the insect meet and greet when he has a phobia of insects" and the response was "he just stopped thinking") and, at some point (near the end of ch1) he mentions that a good way to cope with a bad situation is to just "turn off your emotions with a switch." we don't know if it's something that aligns with him and his way of coping but. it's something to note regardless.

I feel like Kokichi wanted people to not care about his death. I think part of the reason he lied and was so mean was so when he died people didn't care. Like they would react the same way people react to their mum's friend dying like tbeir sorry it happened but they aren't that upset because they never knew them

Kokichi lies even without the context of a Killing Game though. We see that in Salmon Mode & UTDP, his lies ARE a compulsion. He considers stopping lying in his Salmon Mode Harmonious Heart because he wants to get closer to Shuichi, but he immediately backpedals even on that. While I am not a professional, I do have an obsession with V3 and Kokichi especially, and I would also point out the possibility he has PPD (Paranoid Personality Disorder). From what I have researched it is basically a pervasive pattern of distrust and suspiciousness in which other people's motives are interpreted as potentially malicious, including unjustified doubts about loyalty, reluctance to confide, reading hidden meanings into things, holding grudges, and perceiving attacks where others don't. The DSM-5-TR requires at least four of those features and Ouma has ALLLLL them mfs 😭😭😭 his entire worldview is structured around not being able to take anything at face value, he repeatedly questions whether people are telling the truth, whether situations are what they appear to be, whether the others are hiding something, and whether the Killing Game itself is operating according to the rules they think it is, and he's reluctant to reveal anything even remotely personal because he doesn't trust people with information about himself. His paranoia also isn't limited to one person and, arguably most importantly, he doesn't even have a stable reason to trust his own memories and it's pretty inferable he didn't even believe in DICE. His obsession with Shuichi also maps pretty cleanly onto the PPD pattern of being preoccupied with the loyalty and trustworthiness of others (1/2)

Not speaking to anyone in particular this is just a shameless comment about Oma's character because I really like the guy, I haven't even watched this video yet (sorry)! You can argue Kokichi doesn't care about people individually, which kinda, but his main goal is stopping the killing game altogether for EVERYONE. it doesn't really matter what lengths he has to go to in order to stop it because that's how badly he wants it to end. I think while his intentions are morally grey, he's reacting to a horrible situation in a way he has control. Intellectually. He's also stuck with a bunch of erratic + emotionally unstable personalities who cave into pressure easily. Reasonably he's cautious but he's not completely horrible to me and I don't think he ever will be. Maybe it's some inherent bias I have but Kokichi is so gravely misunderstood in his representation it's difficult to capture his likeness. He isn't secretly depressed and he isn't just some manipulative insane guy. If anything he's angry and I honestly wish people would portray him LIKE that more. He's devestatingly honest when it comes to looking at the situation with an unbiased lense. It is horrible and there is no real hope, clinging onto it is like lying to yourself and just postponing pain. He understands they're all being watched and toyed with, it's why he has to keep his distance, run away, watch, and act. So he's being smart, because who wants to be fully honest and intimate with a bunch of strangers who might be willing to kill you for their selfish needs on a whim? Therefore Kokichi disconnects with the people to play a game between him and the mastermind, just like how his opponent views each one of them. To beat someone in power you have to get inside their head, that's just what he does!!!! I'm not a Kokichi apologizer I think but I do see each "contestant" as someone reacting to the situation the best they know how to. This doesn't excuse anything but it gives insight and allows for sympathy + understanding one's actions not just boiling down to an evil act. I think sometimes it's so CRAZY that people are more inclined to feel empathy for TERUTERU a consistent sexual harasser/assaulter..ok?
