@drzackmd: A medical practice declining birth plans that deviate from established clinical standards is upholding evidence-based maternal-fetal safety, not restricting birth support (American College of Obstetricians and Gynecologists, 2019). While doulas offer valuable non-clinical emotional support during labor, board-certified obstetricians must retain the clinical authority to intervene during acute fetal distress or postpartum hemorrhage (Bohren et al., 2017; ACOG, 2020). #DrZacharyMeade #ObGyn #StandardOfCare #LaborAndDelivery #EvidenceBasedMedicine
As a nurse, you have a right to refuse care. However, you don't have the right to demand that I put my license, and possibly my freedom, participating in care that deviates from standards of care.

During my OB gyn residency one of the residents started a study that counted the number of words in a birth plan and plotted it against time to c-section. It was inversely proportional. More words equaled higher chance and shorter time to c-section. It was crazy.

So they’re just going to ignore the important part…IF IT DEVIATES FROM THE STANDARD OF CARE.

My OB said “You can plan all of you want for delivery. However babies have no concept of planning and deliveries don’t always go smoothly.”

Birth plans and culture around it are out of control. Birth is so unpredictable. You need to be flexible and trust in your healthcare team

The birth plan should be to have a baby safely and rely on the professionals to guide that process. That’s it! That’s the whole plan!

People forget what a birth plan is, it’s a list of “hopes” I like to tell people. I’m a NICU nurse and I wanted 2 minutes of delayed cord clamping, but when my baby came out gray and limp with a tight nuchal and they told me they could only do 30 seconds plans changed. And I’d take less delayed chord clamping over a potential hypoxic injury everyday. I do not get this desire to keep absolutely strict to your birth plan over A healthy baby.

Correct. We don't have an issue with birth plans. We have an issue with people putting their lives, their children's lives, and our licenses at risk.

Birth can go wrong. It is a super dangerous period for mothers. Bring your doula, they can wait for you outside the room, sorry! What will a doula do for a cord prolapse, do they know what a McRoberts is or its indication? NOPE NOPE NOPE!

As someone who worked in medical-legal for 20+ yrs this is absolutely smart and necessary. Medical professionals cannot deviate from the standard of care without risking civil and perhaps criminal liability including loss of licensure. Even with all the waivers in the world a doctor or nurse would be insane to agree to anything deviating from the standard of care.

my ob: "so whats the birth plan?"
me: "I go into labor, then you tell me what to do"
ob: "excellent."
turns out I have extremely fast labors, and he almost didnt make it to the delivery in time. any plans I would have had wouldnt have happened anyway, so it worked out well.

When I had my oldest, the hospital wanted me to come up with a birth plan. I was so confused I literally told them “My plan is to birth a healthy baby, and the rest is up to y’all.” Like WHAT DO YOU MEAN BIRTH PLAN? JUST MAKE SURE WE’RE BOTH ALIVE (AND PREFERABLY HEALTHY) AT THE END.

Oh? So if I refuse an epidural and pitocin, and wish to birth naturally, you think I don't have that right? The cascading interventions are often for the convenience of the medical staff and a profit for the hospital.

Mind you, the reason why we have birth plans is because indiction, epidurals and episiotomies were once routine, and cutting the cord immediately after birth, doing the first weigh in and bath, was all done before the mother even got to hold her own baby. Women has to protest for skin to skin and to be able to breastfeed THEIR OWN BABIES immediately after birth at some hospitals. OK so you want to do whatever it takes for you and the baby make it out alive great! That doesn’t tell the hospital staff if you’re planning on rooming in with your baby, if you plan on breastfeeding, how you feel about pacifiers, or if you don’t want visitors while you’re in the hospital etc. any provider who sees birth plans as a challenge IMO shouldn’t be a provider! A birth plan at the very least gives you opportunity to discuss things with your patients.

In the past 12 to 15 years, I have actually seen the term “birth rape” to describe doctors and medical teams who had to deviate from someone’s birth plan. Some of these women are insane.

You wait until delivery to bring up the birth plan, then they can’t drop you as a patient and they need a warrant to limit your choices.
