First of all, the note isn’t exclusive to the weird route, in the normal route it isn’t clarified which people Susie drew. Second of all, Deltarune couldnt have happened before Undertale because characters like monster kid are grown up in Deltarune but a kid in Undertale, also Deltarune couldnt have happened after Undertale because of characters not retaining their memories and because of the note. The only possibility is that Sans goes to an alternate timeline (Undertale)

this would solve why they just appeared but it opens up other holes. why are all the monsters underground exact 1:1s of the surface, yet they claim never to have seen the surface? the monsters in the underground seem established, and have no knowledge of sans and pap before they appear. that would leave open the possibility that sans and pap traversed across timelines and then rooted into the undertale line. this would mean that delta rune and undertale are adjacent timelines, one where the cast is free the other where they are imprisoned. if that's the case, that means that sans and papyrus are somewhat independent of the timelines, possibly being capable of existing outside of time itself. sans upscale
