The throat clearing that will not quit. The lump. The afternoon hoarseness. When allergy meds do not touch it, the reason is often pepsin, the digestive enzyme that rides up with refluxate and keeps irritating your throat, even in a low-acid environment. Here are the first 5 things I would do for LPR. 1. Start a low-acid healing phase. Pepsin sitting in your throat tissue can be reactivated by acidic food and drink and stays active up to about pH 6.5. Temporarily ease off the most acidic offenders like soda, citrus, and heavy vinegar. Koufman (2011, Annals of Otology, Rhinology and Laryngology, n=20 PPI-resistant LPR patients, PMID 21675582) found a strict low-acid diet improved stubborn LPR. This is a healing phase, not forever. 2. Add an alginate raft after meals and before bed. Alginate (a fiber from seaweed) forms a gel raft that floats on your stomach contents and covers the acid pocket, the pool of acid that sits near the top of your stomach after eating, so backflow has a harder time reaching your throat. Rohof (2013, Clinical Gastroenterology and Hepatology, RCT, n=16, PMID 23669304) showed the raft settles onto the acid pocket and displaces it.the one I recommend @RefluxRaft linked in my bio. 3. Practice Mealtime Mindfulness. Sit down, phone away, chew to near applesauce consistency, set your fork down between bites. Slower eating means less swallowed air and volume, which lowers gastric distension and the upward pressure on your LES. 4. Honor the 3-hour buffer. Keep food to 3 hours before bed. Beverages are fine. Lying down on a full stomach makes it easier for refluxate and pepsin to reach your throat overnight. 5. Support your nervous system. Reflux and stress feed each other. In fight-or-flight, blood flow shifts away from digestion, your diaphragm tightens, and LES coordination suffers. Before meals, try my 5+1 Method: 5 slow diaphragmatic breaths plus 1 gratitude. Barillari (2025, PMID 40258993) found significantly higher perceived stress in reflux patients. Follow for reflux science without the fear. #LPR #SilentReflux #AcidReflux #GERD #Pepsin RegisteredDietitian
