Coded my patient who was fine 2 minutes before during bedside. Got them back, sent them to ICU, continued report after (same giving/receiving) then our trach patient decanulated herself right after we left the room and coded too (completely AAOx4, SI attempt in hindsight). ICU was NOT happy to see our faces again literally 10 minutes later. We finished report, charge came to us to tell us our first patient was desatting and they were calling an RRT but knew we were in report and coded two people… 😬. They aspirated. Just a TON of bad luck. My two other new admits also coded (scatter PE, random 3rd degree block). All on tele/MS 6:1.

Not a healthcare worker but a chronically sick kid who within 5 mins of shift change for night shift had a 105.7 fever, white count of 25,000, RBC of 2.92, Hemeglobin of 8, Hematocrit of 23.8, platelet count of 99, BG of 222, sodium of 125, potassium of 2.8, chloride of 94, CO2 of 16, Anion gap of 15, BUN of 27, Crit of 3.65, calcium of 7.3, total protein of 5.3, albumin of 2.7, alkaline phosphate of 36, bilirubin of 1.6, CRP of 19.3, Mag was 1.5, triglycerides were 692, my ast was 48, phosphorus of 2.3, eGFR of 66, CK of 24, Lipase of 947, QTC of 572. I was quickly transferred to the PICU, later was intubated. Also had a AKI. Blood cultures came back within 12 hrs that I was in septic shock with MRSA from a central line infection that wasn’t taken seriously at another hospital I had previously been at 5 days prior. Let’s just say what everyone is thinking. I was screwed and my parents were told to prepare for the worst bc I wasn’t expected to make it through the night

Talked to and gave my patient pain meds 20 mins before after walking her to the bathroom…6:55 next shift asked have her vitals always like that. She was herniating, brain dead by 7:02. Looked back in charting because surely I missed something, even though as a PCU patient I was checking VS Q1 cuz I had a weird feeling….VS changed in two minutes. Like HR from 90 to 25 BP 120 to 250, pupils 3 to 8 and fixed , would have never caught it. This was all after I said around 3:30am to coworkers…”I have never seen a patient herniated and idk what I would do”…well I figured it out 🙁

0645 my DNR patient threw a 11 sec pause and ended up bradying into the 30s. 2 amps of atropine didn’t work, 1/2 amp of epi and went into pulsed vtach. he lived, cardiology wanted nothing to do with him because of his extensive history 😭

Nurse panicked and hit the code button because an a&o4 pt rolled over in his sleep and pulled out his iv so there was blood all over the sheet and side of the bed. NURSE called a code over it. At 6:15.
