Normally, semen initially forms a gel and should liquefy within about 15–30 minutes (WHO reference: within 60 minutes). After liquefaction, it should flow relatively freely. Increased/abnormal viscosity may be associated with: Prostate or seminal-vesicle dysfunction Genital tract infection/inflammation Dehydration Incomplete liquefaction Sometimes obstruction of accessory glands Effect on fertility High viscosity can interfere with sperm movement, making progressive motility appear reduced and potentially affecting sperm transport. Laboratory reporting You can report: Viscosity: Normal Viscosity: Increased/High Liquefaction: Complete/Incomplete If threads are present after liquefaction, note the thread length/formation according to your laboratory's SOP. If increased viscosity is persistent, it is useful to evaluate it together with WBCs, sperm motility, semen volume, pH, and signs of infection rather than treating viscosity alone.
