The pendulum has swung too far toward consolidation. If we want physician-owned medicine to thrive again, we have to address the economic realities that made independence so difficult in the first place. That means stronger collective negotiating power for independent practices, fairer reimbursement for primary and specialty care, lower administrative burdens, more transparent payer contracts, and payment models that reward quality and outcomes, not just scale. AI can also help lower administrative costs by reducing documentation, streamlining workflows, and giving physicians more time to focus on patient care. But technology alone won’t fix the underlying payment and negotiating challenges. Physician autonomy isn’t just a cultural issue. It’s an economic one. I've seen clinics in California going to monthly payment plans for their all cash office. $100 month covers all your pcp appointments and some specialists for the year. It appears to be better for the physicians and patients.

but if the health insurance / corporate medicine industrial complex can hold on just a bit longer they'll have a critical mass of younger physicians accounting for the bulk of practicing docs who will never even known that private practice existed and they'll have so much debt, they won't have a choice but to work for them. it's time for all the doctors to leave the system; we can't afford to wait for the bubble to pop and then take the scraps. we have to show the younger doctors there's something worth practicing for

Every administrator and private equity administrator and investor should be personally responsible to be sued when someone is harmed or dies from lack of healthcare. It’s up to the administrator to prove that what they did was reasonable before a jury. If they’re found guilty, they may be found criminally negligent as well. Couple of executives going to jail are being sued for a millions of dollars might change this whole situation.

Unfortunately, so many podiatry practices have been bought out by private equity groups. Many of them offer low pay to new grads who just completed 7 years of school and residency training.

the only way PE makes money is pay doctors less to treat more pts. employed docs in turn render worse care because why bother. over time, PE group gets a bad rap, independent PP docs like me pick up. it's a lot of work but ultimately I only answer to my accountant and don't take anyone else's 💩

Sadly I don’t think this will occur unless physicians stop undercutting eachother taking garbage jobs and contracts from private equity and large hospital systems. My experience, most of us are out here to get ours and that’s it. We won’t unionize at large. Our medical associations won’t utilize lobbying like PE firms, hospitals, and insurance companies. We need a large grassroots change. I just don’t see it happening.

After being employed for 18 years by hospital as infectious disease specialist, the hospital closed down, forcing us to go to private practice. Currently, I’m doing only inpatient to avoid overhead.
