@doctorhaleymain: “Your hips are out.” We hear this ALL the time when someone has low back, pelvic, or SIJ-region pain. But instead of assuming the pelvis is simply “out of alignment,” we need to ask a better question: Is the SI joint actually the pain generator? SIJ pain can overlap with symptoms coming from the lumbar spine, hip, and surrounding soft tissues—which is exactly why your examination and clinical reasoning matter. Rather than chasing alignment, use a cluster of findings to help determine whether the SIJ is actually contributing to your patient’s symptoms. 📩 Want my SIJ Pain Generator Quick Guide to help you identify these patients faster? Comment “SIJ” and I’ll send it to you. And if you’re a PT who wants to get better at identifying pain generators, building a treatment plan, and knowing what to do next with your patients, that’s exactly what we work on inside my Foundations to Clinical Success Mentorship. Comment “MENTORSHIP” and I’ll send you the details. #physicaltherapy #physicaltherapist #SIJpain #sacroiliacjoint #lowbackpain

Schools do a terrible job of providing students with tools and clinical reasoning. I have never once palpated for PSIS to find any useful data once I graduated. Also, regardless of what the research shows, in practice, the thrust test has consistently been the only clinically relevant test in that cluster to the point that I typically skip the others.

Waste of time. Just check for function during palpation. These tests are good but wastes your time unless it’s part of the treatment. Palpating psis and watch for movement bilaterally during forward flexion and extension.

True SI dysfunction is rather rare, check the hip and lower back first to rule those out

What about Gaslean and FABER'S test?
These tests are also provocative tests for SIJ dysfunction.
