@drj1189: Reaching your goal weight doesn’t automatically mean it’s time to stop your GLP-1. Weight regain after discontinuation is common. In clinical trials, stopping semaglutide or tirzepatide has been followed by recurrence of a substantial portion of the weight lost, along with reversal of some cardiometabolic improvements. The 2026 ADA Standards of Care now specifically recommend continuing obesity pharmacotherapy beyond weight-loss goals when indicated, with the long-term plan individualized to the patient. That plan may look different from person to person. It can include continued pharmacotherapy, an individualized maintenance dose or strategy, or discontinuation with close monitoring and a thoughtful plan for nutrition, physical activity and resistance training. We also have a newer option to discuss: oral orforglipron. The phase 3b ATTAIN-MAINTAIN trial directly studied people transitioning after weight loss with injectable semaglutide or tirzepatide and found that orforglipron preserved substantially more of their previous weight loss than placebo. The conversation I want patients to have with their physician isn't simply, “When can I stop?” It’s “What is my maintenance plan?” Losing weight is one phase of obesity treatment.
