BiPAP = you set the pressures. AVAPS = you set a target tidal volume, and the machine adjusts pressure support to help reach it. AVAPS can be useful when you need more consistent ventilation, but it’s NOT magic. Mask leak, poor synchrony, secretions, worsening respiratory failure, and patient fatigue can all make NIV fail. And remember the most important part: don’t let a fancy NIV mode delay intubation in a deteriorating patient.
