We all need to refuse to participate in the unsafe demands that have become common. NO - I will not see more than 3 patients per hour in primary care. NO - I will not respond to portal messages without pay unless we do down to 2 patients per hour. Unacceptable. All of it.

When you are a new clinician, it’s very hard to understand what is a reasonable expectation, so calling it out is so difficult…and we look to those exact people to lead us. We look to science, facts and evidence to lead us in practice-among empathy & compassion. When that get mixed with money it’s almost like there is no meeting point that is the gray area we live in. 😔

Where are these disciplinary actions coming up for future employers and licensure? As a manager, I never saw any dings but Board or State dings. (Not suggesting they don’t exist, rather, wondering if I’m not familiar with a system or process that I should be.)

so much this... first job out of residency at a FQHC... half my panel is npw addiction med on suboxone, treat 5-10 hep c patients a week, manage their primary care, also do like a chronic pain management clinic with chronic opioids, helped with their hep c/hiv protocols, take walk ins for substance use at any time with a full schedule... I didnt learn how to do most of this shit during residency... have learned a ton... asked for a raise and nope basically got a "anyone could do your job" i said this is so stressful and chaotic, go find them and am leaving in November lol

My sister's favorite doctor ever took over her dad's practice when he retired. As she got older she decided to go with the hospital system that had been trying to lure her in for years. Largest system in the pittsburgh area. there was never more than a 5-10 minute wait in her office and she spent a lot of time with her patients. By the second or third year she was fired for not bringing in enough revenue. It was a disgusting situation, and actually from what I hear it still is.

Yep! Not my husband's old employer doing it to his colleague. The man is working for practically free now. To the point where my husband took Doc aside and said, " If you ever need testimony to get out of this situation, I'll be happy to write up what I saw. Because man, that guy is tired and trapped. And his patients are suffering for it.

This reminds me of the practice of some co-workers when I was a Visiting Nurse of under reporting their hours. For some reason, be it feelings of unworthiness or insecurity, many were seeing all of their patients, but not documenting the time it took accurately. We on the co-governing board did an anonymous survey, and more than half were cheating themselves. It also made those of us accurately documenting our time look inefficient. 😬 Pressure!
