@radiology.technology: 🧠 4-Step Approach to Detect Acute Ischemic Stroke on CT Brain When reviewing a non-contrast CT (NCCT) brain in a suspected acute stroke patient, these 4 simple steps can help you systematically look for early ischemic changes: 1️⃣ Compare Both Brain Hemispheres Start by comparing the right and left cerebral hemispheres. Look for asymmetry, swelling, loss of normal sulcal spaces, or subtle differences in attenuation. Comparing both sides can make early abnormalities easier to recognize. 2️⃣ Look for the Hyperdense MCA Sign Carefully inspect the Middle Cerebral Artery (MCA). A hyperdense MCA compared with the opposite side can be an important early CT sign of an MCA thrombus/occlusion. Always interpret this finding together with the clinical picture and other imaging findings. 3️⃣ Check Gray Matter–White Matter Differentiation Look for loss of the normal distinction between gray matter and white matter. Early ischemic change may cause decreased attenuation and blurring of this differentiation, particularly in areas such as the insular cortex and basal ganglia. 4️⃣ Look for Early Ischemic Changes & Their Extent Finally, assess for focal hypoattenuation, sulcal effacement, and loss of normal anatomical landmarks. Determine how much of the brain territory appears involved. In suspected anterior-circulation stroke, systematic assessment can also support clinical decision-making and further evaluation. 📌 Remember: Early ischemic changes on CT can be subtle.
