The real clue in this scene is the gigantic coat he’s wearing bc it’s not his coat, it was given to him to play the part in his mind … the movie definitely goes the direction of he was in fact mentally ill but was self aware in the end and chose to be “put down” if you read the novel however it’s 50/50 whether he was crazy or if they gaslighted him into feeling crazy so he couldn’t leave and blow the lid off of shutter island and their methods of handling patients. If you haven’t read the book, I advise you do
