@tracypekurny: I talk to nurses every single day, and one of the first questions I ask is: What exactly are you planning to do for your clients? Because more often than not, they start describing a business that crosses between what I call Path A and Path B, and they don’t even realize they’ve crossed the line. They’re thinking, I already do this in the hospital. I’m an RN. Why couldn’t I offer it privately? But taking a clinical skill outside the hospital and turning it into an independent private-pay business can change the infrastructure required around that service. There are generally two paths I teach, and sometimes a hybrid between them. Path A is the simpler concierge nursing model, built around services that can be appropriately structured within the RN’s applicable scope and state requirements. Path B is where the service itself may trigger additional regulatory requirements depending on what you’re doing and where you’re practicing. That could mean medical oversight, prescribing or ordering authority, additional licensing, agency requirements, pharmacy considerations, another regulatory board, or a very different legal structure. And Path B isn’t bad. It can be an incredible business. But it may require more time, capital, infrastructure and resources than the nurse anticipated. That’s why I don’t start by asking, “What would be the coolest business you could build?” I want to know what makes sense for your energy, your available resources, your financial capacity and the life you actually want this business to support. Sometimes we start with Path A and eventually scale into Path B. Sometimes Path A becomes an incredibly profitable business and we never need Path B. Sometimes the nurse has the resources and vision to build Path B from day one. The important thing is that we know which path we’re building before we start building it. Because the worst time to discover that your business requires additional oversight, licensing, capital or infrastructure is after you’ve already created the offer, priced it, marketed it and started spending money. This is also why legal education is built into my Cohort. I work directly with healthcare attorneys who help my nurses understand the legal and regulatory considerations surrounding what they’re building and, when individually retained, can help establish the appropriate legal infrastructure for their specific business. I want nurses building businesses without operating from the constant fear of: “Someone told me I’m going to lose my license.” Fear isn’t a business strategy. Neither is assuming your RN license automatically makes every business model permissible. Clarity first. Structure it correctly. Then go build the business. That’s how we turn an idea into a bona fide concierge nurse business that can actually support the life, income and impact you’re trying to create. last Cohort of 2026 starts October 13. If you’re a nurse that you know you’re starting a business and wanna help building it out, Cohort and I’ll send you the details.
