That swing is not that of a 19 hcp. Improve your course management and short game. And if there is a particular hole that you always slice, it’s more to do with the way the hole appears visually to you causing you to setup a certain way or swing a certain way. Just set your aim once and then setup to the ball and ignore the tee box visuals. This happens to me on tee boxes that are purposely aligned off center. Just block it out.

You could simplify and just close the face at address with your grip. You can change your path at address in your alignment, so you don’t swing any differently. Path + face orientation at impact will determine initial ball flight direction followed by the flight due to spin created due to the face orientation at impact.

Love the swing thought. If you move your left toe an inch toward the ball and your right toe away an inch, this will help the swing plane a little.
