Get rid of insurance, all services should be self pay. Costs are high because there is no true competition as insurance and big healthcare networks set all the prices. Catastrophic insurance should be provided by the government with a tax on wages and based on a multiple of income. Healthcare costs should be 100% tax deductible. Get rid of this nonsense penalty at the state level for not having health insurance.
