Doctors have a pretty strict confidentiality code of ethics. An SW is probably a grey area as far as conflict goes, but he’s probably right to refer you to a colleague. He doesn’t have to provide any detail besides say there’s a conflict. There would also be very severe penalties if he were to use any of your private information. I’d be very surprised if he actually booked you again after his interaction. There’s probably a similar fear in him that you now know where he works and can easily get access to his colleagues.

Doctors are bound by the Hippocrates oath? (can't remember the exact wording) so they are bound by strict patient confidentiality rules so I think you'll be fine.

I have a bit of faceblindness often mistaking siblings of celebrities for yeah bigger sometimes celebrities and yeah who knows definitely I would even confuse mcconaughy & harrelson SO it will be weird if they announce they’re not actually biological brothers🤯

I remember people mostly by their hair styles. So if the change them and we know each other I _immediately_ notice. But if I don’t know them well, it might not be able to remind myself who they are. 🙈 I’ve already learned to not make it more weird than it is and just ask directly. And no one is angry about it. People seem to _appreciate_ that you want to remember them. Of course in your particular case it is probably for the better, but as you say it might be good to look them up before the visit.
