What is SHBG and why do I care when I see it on your blood test? SHBG stands for sex hormone binding globulin. It’s a protein produced predominantly by the liver that binds to sex hormones in the bloodstream, particularly testosterone and oestradiol, and helps transport them around the body. But here’s the clinically interesting part. When testosterone is tightly bound to SHBG, it isn’t readily available to tissues. So knowing someone’s total testosterone alone doesn’t necessarily tell me how much testosterone is biologically available. Think about two women who have exactly the same total testosterone result. One has very low SHBG. The other has very high SHBG. Those two women can have very different amounts of available testosterone and potentially very different clinical pictures. This is particularly relevant in conditions such as PCOS. Low SHBG can mean a greater proportion of testosterone is available, which can contribute to signs of androgen excess such as acne, increased facial or body hair and irregular menstrual cycles. But low SHBG can tell me something else as well. Insulin suppresses hepatic production of SHBG. So when I see low SHBG alongside other metabolic abnormalities, I start asking whether hyperinsulinaemia or insulin resistance could be part of the picture. That does NOT mean low SHBG diagnoses insulin resistance. It doesn’t. SHBG is influenced by multiple factors and needs to be interpreted in context. At the other end, high SHBG means more sex hormone is bound, potentially leaving less available to tissues. SHBG can be influenced by oestrogen exposure, thyroid function, liver function, metabolic health, medications and other physiological factors. This is exactly why I don’t interpret SHBG as an isolated number. Depending on the person, I may look at SHBG alongside total testosterone, the free androgen index, oestradiol, thyroid markers, liver markers, glucose and insulin metabolism, menstrual history and the symptoms sitting in front of me. And this is the bigger lesson when you look at pathology. A blood test isn’t simply a collection of individual numbers that are either “normal” or “abnormal”. The relationship between those numbers can sometimes tell us far more.
