What if your AMH says your ovarian reserve is low, but your ultrasound tells a different story? This is exactly why I don’t like AMH being interpreted as a standalone fertility marker. AMH is produced by granulosa cells within developing follicles, particularly preantral and small antral follicles. So we’re not directly measuring how many eggs you have left. We’re measuring a hormone that gives us an indirect marker of ovarian reserve. Antral follicle count, or AFC, looks at ovarian reserve differently. During an ultrasound, the small follicles visible within the ovaries are counted. Often AMH and AFC correlate reasonably well. But sometimes they don’t. You might have a relatively low AMH but a more reassuring AFC. Or your AMH may appear higher than expected for the number of follicles seen on ultrasound. When those numbers don’t match, I don’t simply choose the result I prefer. I start asking why. Was the AFC performed at the appropriate point in the cycle? Could hormonal contraception be influencing AMH? Is there a history of ovarian surgery or endometriosis? What are the other fertility markers telling us? And there’s another distinction that is incredibly important: Neither AMH nor AFC is a direct test of egg quality. They primarily give us information about the quantity of the remaining follicular pool. A low AMH does not automatically mean the eggs remaining are poor quality, just as a high AMH does not automatically mean you have a higher chance of conceiving. This is why fertility assessment needs context, not one isolated number.
