@queeny540: Can you document a patient’s exact swear words? Yes—but only when those words are relevant to their care, behaviour, risk or an incident. Use quotation marks to show that they are the patient’s own words, document the surrounding behaviour objectively, and record the action taken. Avoid including offensive language unnecessarily, and always follow your organisation’s documentation policy. If it’s not documented, it didn’t happen.#healthcaredocumentation #healthcareassistant #nursingdocumentation #supportworkers

I document word for word. Patient used profane and abusive language towards me, stating, “Fuck off. I do not want to see your face. I hate you. You look like an animal.”

If you don’t document exactly what happened and something happens, you cannot defend yourself properly. Documenting exactly what happened helps to protect yourself in future events when they need to establish a patient’s behavior and how to support them.

I do particularly if a racist comment. I put it in speach quotes as it is important to show the reason why they were either made to leave my clinic room or why I ended a call. You know, ready for my response to the PALs department.

I will ALWAYS write word for word what they said and put it in quotation marks. I'm not just writing patient swore, im writing 'patient began to get agitated, walking towards staff, patient picked up the lamp on the table and began waving it at staff saying 'im going to f'cking cut you up b!tch like your husband should have' 🤣 90 year olds be absolutely crazy when they get going.
