@crnaschoolprepacademy: A central line may feel like the most powerful access in the room, but central placement does not automatically mean high flow. A standard central venous catheter is long, and each lumen may have a relatively small internal diameter. Both characteristics increase resistance. A short, large-bore peripheral IV may deliver fluid faster because flow is strongly influenced by radius and inversely related to length. Poiseuille’s law gives us the framework: Flow ∝ pressure × radius⁴ ÷ viscosity × length The fourth-power relationship is the key. Small changes in internal radius can create large differences in flow. The rest of the system matters too. A long extension set, stopcock, needleless connector, viscous blood product, kink, or partial occlusion can become the rate-limiting component. One important distinction: a standard multilumen central line is not the same as a large-bore introducer or catheter specifically designed for rapid infusion. Do not ask only, “What gauge is the IV?” Ask: • What is the internal diameter? • How long is the catheter? • What fluid am I infusing? • What tubing and connectors are attached? • What flow rate is the device designed to deliver?
