I was inpatient post- L4-5 fusion and reading the chart notes via MyChart. One day my nurse said, "Wait, you're really reading those notes!?". I said, "Definitely. I skimmed them and that's how I knew to suggest providers discuss with their colleague who stated I experienced an isolated fever (102.4) that she needed to correct it w/o a HIPAA amendment because the factual data in the record (I.e., multiple 100.4+ temps after the 102.4) didn't agree with her visit note.

Why would a health care provider inaccurately document a patient’s chart? I know it happens; I’ve experienced it myself.

Yes! My MD charted that I was asking for daily opioid management for pain and pain clinic refused. The truth was the pain clinic wanted to put me on daily pain meds and I refused the contract because I didn't want that. The thing that made this especially bad was we never had a conversation about this during the appointment and this was her whole progress note. I told her I saw her charting make derogatory statements about pain medication and we never had this discussion during the appointment. And asked her why would she chart something that wasn't true and can give others the wrong opinion. When is comes to chronic pain management this happens so much.

12 years of being labeled anxiety and then panic disorder. I have cervical myelopathy, cervical and formainal stenosis, hEDS, MCAS and audhd and that's just what I know recently. It's the constant erasure that's caused so much trauma. It's like moving through the world and always feeling like an alien and being blamed for it.

when they find out you read the summary notes it changes the game. im finally being heard ( that reduces patient stress)..and everyone can hear me now. you are 100% correct. Also quotations work great in documents and should be used more. it doesn't have to make sense what you say to the dr but it might to another Dr that reads that and can help with continuity of care.

Just one of countless reasons why I don't participate in the Healthcare system anymore. Short of a video appointment once a year so my doctor will keep prescribing my high blood pressure meds and acid reflux meds.

This!!!! I found the neurologist I was seeing and expressing my problems to was reporting that I was fine and the meds were working. They were not working and I was not ok. Fired her once I read her notes

I asked for my file and was refused. When I pushed they made it impossible with a bunch of hoops to get through to get my papers. Like what are we hiding? why are they so upset. I believe my care is effected by notes. I'll have to keep trying now that I have a new doctor, but I found it out its so secretive when its my right.

I had an amendment added to my record, but was told Care Everywhere doesn’t transmit the amendment or Statement of Disagreement and the next provider may never see your correction.

mind blatantly lie. they make things up they add their own interpretations of things that I have expressly defined and they dismiss and downplay all of my symptoms as drug seeking. as I lose a half inch of height from spinal collapse and as I am down 11% of my body weight in a year. they're all so ignoring that both my kids have the same disease and symptoms but they keep taking our diagnosis and trying to make it child abuse and psychiatric
