Nothing clinical changed. The paperwork changed. The numbers on this are not subtle. Medicare pays roughly $475 for an MRI at an independent imaging center and roughly $720 for the identical scan at a hospital outpatient department. On the commercial side the gap is worse, independent centers commonly run $400 to $1,200 cash while hospital outpatient departments run $1,500 to $3,500 for the same images. Freestanding centers do not add a facility fee. Hospital outpatient departments do, on top of everything else, and the radiologist often bills separately too. Same equipment. Same pictures. Same person reading them. Two to five times the price depending entirely on who owns the building. This is also why independent doctors keep disappearing. If a hospital gets paid more than an independent practice for identical work, the independent practice cannot compete on those terms. It is not a fair fight, and it was never designed to be one. Competition is what drives quality up and prices down, and this arrangement removes the competition and calls it consolidation. The deeper problem is what actually makes a healthcare business successful in this country. It is not being good at medicine. It is being good at billing insurance. That is where the money is. So when you see a wildly profitable healthcare organization, that is not evidence they deliver excellent care. It is evidence they are excellent at extracting money from claims.
