@nourishingapothecary: A hysterectomy is NOT a cure for endometriosis. This is one of the questions I’m asked most often in clinic, and it’s also something I can speak about personally because I’ve had a hysterectomy myself. For me, it was the right decision. But I still have endometriosis. Why? Because endometriosis is not simply a disease of the uterus. Endometriosis lesions are tissue similar to the lining of the uterus growing outside the uterus, and removing the uterus does not automatically remove all endometriosis lesions elsewhere in the body. This is also where endometriosis and adenomyosis need to be distinguished. Adenomyosis occurs within the muscular wall of the uterus, so hysterectomy can be definitive treatment for adenomyosis. Endometriosis is different. That doesn’t mean hysterectomy is never appropriate. For some women with severe symptoms, adenomyosis, heavy bleeding or other gynaecological issues, it can dramatically improve quality of life. But it is major surgery, and the decision is highly individual. What is removed matters too. A hysterectomy removes the uterus, while removal of the ovaries is a separate procedure and has very different hormonal consequences. This is why I don’t believe there should be a blanket answer for every woman with endometriosis. You need to understand your disease, your symptoms, your fertility goals and your options so you can make an informed decision with the right healthcare team around you.
