@dr.zac.spiritos: Once you diagnose MCAS, that can actually be the easy part. The harder part is often figuring out how to treat it. We have medications that, in theory, should help stabilize mast cells or block the mediators they release. The operative words are in theory—because mast cells do not always follow the playbook. Someone may react to the medication itself. They may react to a dye, filler, preservative, or another excipient in the formulation. And for someone who has had repeated bad experiences with medications, the stress and anticipatory anxiety around taking something new can sometimes amplify symptoms even further. So getting medication into an already highly reactive system can require a lot more troubleshooting than simply picking the “right” drug. Sometimes that means compounding a medication to remove unnecessary additives or starting at a fraction of the usual dose. A liquid formulation can make it much easier to start extremely low and slowly work upward. Other times, we switch medications entirely, change the formulation, or even change the route of administration—occasionally an injectable medication is tolerated better than an oral one. This is why MCAS care often requires close follow-up and a willingness to adjust the plan. The diagnosis gives you a framework. The real work is figuring out what that individual person’s body will tolerate, finding a way to get treatment on board, and slowly building from there. #mcas #dysautonomia #heds #pots #mastcellactivationsyndrome

Me over here after having a severe anaphylactic reaction to Xolair and ending up on high dose prednisone for months trying to get my body to calm down to find out 6 months later that it has an ingredient derived from coconut which I am allergic to. The best medication that we’ve found that has helped with my MCAS is Cellcept

Ketotifen for me 😭 it was helping but then gave me the WORST rage of my life. I tried to wait it out but it got worse. They took me off of it once they had 2 others call saying the same thing was happening to them.

I was told getting a diagnosis for MCAS is impossible where I live / so I just started treating it myself. Pepcid ac twice a day and Claritin nightly. Works like a charm for my pmdd also.

The change from Mylan to Mint-famotidine was the worst month of my life. From one generic to another and some how I spent a whole month with nausea so bad I thought I was dying. Then I said hey… didn’t my brand change a bit ago. Because every other option has been cleared, there was no reason for nausea. I went and bought the brand name off the shelf and the nausea was gone by the next morning. I now have to explain to my doctor the need for a “no-sub” on a medication that shouldn’t need it. 😂 MCAS is so weird.

the random reactions to meds you've taken forever is so stressful. I recently had anaphylaxis due to plain tylenol...

One tiny change of an inactive ingredient and suddenly I’m covered in hives. Found the perfect med that helped… until insurance changed sources. Back to the only two meds I can tolerate for unrelated issues. Ugh. I HATE this.

normally I'm so against medication because I have such adverse paradoxical reactions to them. I recently started taking an ADHD medicine stimulant, and it was going well until it wasn't. and now I'm reacting to the medication. but it does such beneficial things like helping me with dopamine eating, but I'm also getting bradycardia and sleepiness and I feel like I'm on a boat after taking it and then I get really sleepy and then I get rashes and then it just tunnels downwards and it's sad because this is the first medicine in my entire life that I actually want to stay on

funny thing: my l-tyroxine, that i need to take EVERY DAY FOR THE REST OF MY LIFE since i got my thyroid out last year because of cancer, is one of my worst mast cell triggers 🤡
we tried other brands, but they were worse. So now i am on bilastine and probably will stay on that for idk how long and was also on famotidine for months, just because i keep reacting to the synthetic hormone, or at least it triggers reactions. Currently i am stable enough so that we could pause famotidine, but its still real fun ✨

I am soo relieved that I am not the only one! I’ve had a bad weekend so am now fasting. Once my system calms down I am going to try aloe Vera juice. I can’t take supplements-allergy meds have nasty side effects. I am lucky enough to not have anaphylaxis-at least at this point.

And this is assuming you can find a team of doctors willing to prescribe said medications. So many doctors don’t like the idea of working with a medication “outside of their wheelhouse” even if they know that it has the potential to be beneficial. MCAS is such an exhausting condition to live with.
