When I assess someone’s fertility, I don’t just look at their blood tests. I want to understand what is happening across their entire menstrual cycle. Are you ovulating regularly? How long is your cycle? What is your bleed actually like? Is it unusually light or heavy? Are there clots? Do you experience ovulation pain or spotting? What happens to your symptoms throughout the month? Because your period is the end result of a whole sequence of events that happened earlier in the cycle. Follicles need to develop. A dominant follicle needs to emerge. Ovulation needs to occur. The corpus luteum then needs to produce progesterone to support the luteal phase. So when something about the cycle looks unusual, I want to work backwards and ask why. Even something like a very light period, which many women assume is a good thing, can sometimes give us information about what happened earlier in that cycle. This is also why pathology is only one part of a fertility assessment. Blood tests are incredibly useful, but they are snapshots in time. Your cycle history and symptom picture can add clinical context that a single blood result simply cannot give us.
