Normally, when we think about blood sugar dropping after eating, we think about things like dumping syndrome—where food moves too quickly from the stomach into the small intestine, causing rapid carbohydrate absorption, a large insulin response, and then a blood sugar drop, typically 1–3 hours later. But that’s not always the pattern I see with MCAS. I’ve seen people whose blood sugar starts dropping within minutes of eating, without the typical timeline we would expect from dumping syndrome. The pathophysiology here is still relatively unclear, but these episodes can be scary and cause shakiness, weakness, dizziness, palpitations, brain fog, sweating, or feeling like you might pass out. Fortunately, I’ve also seen this improve significantly with targeted therapy. A lot of people ask, “What should I take for it?” There really isn’t one answer. Treatment is highly individualized and depends on the person, their triggers, their glucose pattern, and what else is driving their symptoms. My mom has diabetes and I was testing my blood sugar with her and it was super low blood sugar even though I had just eaten and I noticed it a few more times and it made a lot of things make sense. I've always said if I eat anything sweet on an empty stomach I feel like absolute shit.

For those in the comments, it isn’t always food intake that causes blood sugar regulation issues. It could be from exercise, adrenaline surges, illness, stress, pain, your hormones, taking a really hot or really cold shower, etc. So while you may be meticulous with your nutrition, blood sugar issues can still happen.

I have hypoglycemia..... I also recently found out that low sodium causes hypoglycemia symptoms too. which explains the one time I had lunch. only to have severe symptoms an hour later, like blurred vision and feeling like inwas going to faint... and it only went away after having something salty..... just found out I have MCAS, and EDS among some other autoimmune disorders

Yes! Especially in the morning about 20- 40 minutes after breakfast. It’s weird it’s not all the time just when I’m in a “flare” sometimes I can eat anything just fine and other times just a little bit of carbs totally makes me crash out bad.

I thought I was diabetic, but my A1C was normal. I ordered a cgm and found I was spiking and crashing within 45 minutes of eating. I would spike fevers with the crashes (102) almost daily. Exhausting.

Had a continuous glucose monitor. My endocrinologist stared at my low glucose charts for ages and kept asking “and you ate?”. She has zero knowledge of MCAS (like most docs) and was convinced it was not related. She was one of 40+ that couldn’t help me

Yep I have this. I have to eat long acting fibre a half an hour before any exercise, so I have those breakfast biscuits (200 calories for 4 of them) and fruit, banana and two small oranges. That I replace breakfast and lunch with, then have a normal dinner. It stopped the dumping altogether, stopped the crashes and lows.

Legendary protein pastries , and quest protein cookies before or after meals completely made my reactive drops stop happening. They’re just expensive , but it’s the only thing that worked and I haven’t reacted in months . Even complex carbs and peanut butter would shoot me up and down, I couldn’t eat anything before .

Ok so I’ve finally been diagnosed with ehlers danlos, MCAS, POTS (and celiac) but when I was a kid I would have fainting spells (often I’d puke then faint or just faint) and no dr could figure it out and just said I had hypoglycemia. One of many misdiagnoses over 30yrs
