Not every knee lacking extension has a cyclops lesion. But in my clinic, it’s something I want to rule out until I’m happy there’s another reason. It really frustrates me seeing ACLers being told to keep pushing, keep stretching, keep forcing it straight for months on end. If you’re consistently doing your quad sets, several times a day, lying AND standing, you can see and feel your quad fully contracting… yet that knee still won’t straighten or keeps tightening back up?I want to know WHY. Some people can have a cyclops lesion and never even know it’s there. Full extension. No pain. No symptoms whatsoever. But persistent loss of extension, swelling, anterior knee pain or that clicking/clunking feeling deserves investigating — not months of simply trying harder. Like I always say in clinic: You know your knee best. If something doesn’t feel right, speak up. And if you don’t feel listened to, find a therapist or doctor who WILL listen and investigate. An MRI can help identify a cyclops lesion. Sometimes you don’t need more rehab. You need to know what’s stopping the rehab from working.
