@doctoradamhoward: CLOZAPINE is the most dangerous and powerful antipsychotic we have. Despite a remarkable side effect profile, including myocarditis, increased seizure risk, severe constipation, excessive drooling, and an interaction with cigarette smoke that can result in a sharp decrease of clozapine levels, it can achieve miraculous results in some patients. As psychiatrists, most of us in the United States reach for Clozapine only after a patient has failed to adequately respond to two or more lower-risk antipsychotics. In such cases, it can be remarkably beneficial, even restoring some patients who are too sick to leave the hospital to a state where they can return home. Because of the remarkable benefits of clozapine, some psychiatrists and pharmacists are of the opinion that our prescribing patterns are too restrictive. Had a patient that had been on it for years, came in because he had agranulocytosis... it was originally thought to have been caused by the medication, turns out it was Leukemia (discovered 6 months later when IP RN's continued to push the docs to look outside of "clozapine is the cause ")

It's interesting how Australia utilizes the drug so much more than the US. Some reasons are likely due to the fragmented healthcare system in the US versus Australia's health care system that is more structured when it comes to community mental health and long term monitoring.

Don’t forget to tell them about bed wetting. It happens SO often and pts are so embarrassed and never told about the side effect!

Previous inpatient psych nurse here: Clozapine was a life changer for quite a few. Provided they had the adequate resources to ensure they got the required monitoring of this drug.

The psychiatrist I work with loves this med!! We call her the Clozapine Queen. It makes SUCH an incredible difference in the right patients, and I love seeing them transform from their psychosis to being stable and rational on the right medication.

We had a Patient on our Forensic ward, who would have benefited from Clozapine, but she was happy in her own wee chaotic world and becoming well on this medication may have given her too much insight into her illness
