Assalamualaikum, The patient is a male aged 65+ years with a history of diabetes mellitus (DM), hypertension (HTN), and triple-vessel coronary artery disease (TVCAD). For the past 7 to 8 days, he has been experiencing significant pain in the left shoulder blade/wing area, particularly at night while lying down or just before going to sleep. The pain is sometimes severe enough to prevent him from sleeping and usually lasts for a maximum of around 30 minutes. The pain is relieved after taking Sinaxamol Plus. There is generally no pain during the daytime, and the pain does not increase with lifting or moving the arm. An ECG was performed, which showed no significant changes apart from the known cardiac condition, and was otherwise reported as normal. We have already consulted a doctor, who suggested that this type of pain can occur at this age and may be related to diabetes. Could anyone please advise what could be causing this type of pain? Could it be musculoskeletal, or should we be concerned about a possible cardiac cause given his history of TVCAD, diabetes, and hypertension? Any guidance or relevant experience would be greatly appreciated.
