Overnight highs are definitely one of our biggest challenges right now. Hormones can be so tricky to manage, especially on MDI. Aviya's insulin needs are constantly fluctuating, and sometimes her basal just isn't enough to keep up. We've learned her patterns pretty well, and if she's above 120 and showing a pattern of continuing to rise, I'll usually decide to correct. Yes, I correct a 120 overnight 😱 But sometimes I'll turn off an alarm to give it a little more time, only to wake up later and realize the high has gotten away from us. By then, we're dealing with a much more resistant high. It's frustrating! One thing that makes overnight checks so much easier is alternate-site testing. We use the genteel lancing device which is virtually painless and allows me to get a blood sample from her anywhere without moving her or disturbing her sleep. She barely notices! It also gives her fingertips a break. While fingertips are best when glucose is changing quickly or insulin and food are actively affecting blood sugar, we've found alternate sites to be very accurate overnight. I've compared them many times with fingertip readings and her Dexcom.
