@drmresident: We always want to support our colleagues in other specialties however we can. In addition to physical and occupational therapy, this person should also be offered long term psychotherapy as part of their plan. But in the acute hospital setting she needs someone to hold her hand, listen, and grieve with her. This could be done by the surgical team, a social worker, nurse, psychologist, chaplain, or psychiatrist. This likely isn’t a difficult diagnosis and likely does not require medications in the short term. #psychiatrytiktok #medicine #MentalHealth #residency

I actually work in an ID clinic and so many of our virologists want psych consults for newly diagnosed HIV patients because “they seem upset.” Like…obviously?

I once got consulted because a woman was crying persistently. When I got there she told me she just had another miscarriage. They just needed to sit with her and listen.

I wish people would utilize chaplain services more often! They aren’t just there for religious support. They are trained on how to support people through emotional trauma and are such a wonderful resource. Situations like these call for chaplain resources, music therapy, or pet therapy consults. Not all facilities have those resources, but many of them do and people just don’t utilize them

Always getting referrals for “pt in crisis” but then it’s just a person having reasonable human emotions/ behaviors to a specific scenario.

Surgeon here. We know losing a limb is devastating and that grief is normal. The issue is that patients often need repeated reassurance. Hardly any of us inpatient physicians in any field have that time. That’s part of why we have a multidisciplinary team of PT/OT, the chaplain, psych, and my personal favorite, Nemo, our hospitalist therapy dog. Especially for us, being in a rural area, that’s about all we have in terms of resources. We don’t have therapists or psychologists. Would be nice though.
We know Psych isn’t there to be a therapist, but the title “psychiatrist” carries weight. Having reassurance from a psychiatrist can land differently than hearing it from the surgeon who just operated on you. We appreciate all that you do. And we are just trying to do what is best for our patient with the limited healthcare resources that we have.

My teenager went through leukemia treatment and toward the end of treatment she had a psych consult and the doctor said “and of course she’s depressed… she’s 15 and done 3 years of cancer treatment.” It honestly flipped a switch in my head
