Your thyroid could be affecting your fertility, even if you’ve never been diagnosed with a thyroid condition. When investigating fertility, thyroid function is one of the areas I pay particular attention to. And it’s not just about whether your TSH is within the laboratory reference range. Thyroid hormones influence metabolism, menstrual cycle regulation, follicular development and ovulation. But one of the most interesting connections between thyroid function and fertility involves a hormone called prolactin. Here’s why. Your thyroid is regulated through the hypothalamic-pituitary-thyroid (HPT) axis, while your reproductive hormones are regulated through the hypothalamic-pituitary-ovarian (HPO) axis. These systems are interconnected. When thyroid hormone production is insufficient, the hypothalamus may increase production of thyrotropin-releasing hormone (TRH). TRH stimulates the pituitary gland to release thyroid-stimulating hormone (TSH). But TRH can also stimulate prolactin. Prolactin is best known for its role in breastfeeding, but when levels become significantly elevated outside pregnancy and lactation, it can interfere with reproductive function. Elevated prolactin can suppress gonadotropin-releasing hormone (GnRH), affecting the release of LH and FSH, two hormones essential for follicular development and ovulation. This means untreated hypothyroidism can sometimes contribute to irregular cycles, anovulation and difficulty conceiving through more than one pathway. It’s one reason thyroid function and prolactin may need to be considered together when investigating irregular ovulation. But what about women whose TSH is technically normal? This is where interpreting pathology correctly matters. TSH is generally an excellent first-line marker for assessing thyroid function. However, depending on symptoms, medical history and previous results, additional testing such as free T4 or thyroid antibodies may be appropriate. Thyroid antibodies can help identify autoimmune thyroid disease, including Hashimoto’s thyroiditis. But having thyroid antibodies doesn’t automatically mean your thyroid is underactive or that they’re responsible for fertility difficulties. Similarly, a normal TSH doesn’t mean every woman needs extensive additional testing. The clinical context matters. And thyroid function becomes particularly important once pregnancy occurs. During early pregnancy, the developing baby relies on maternal thyroid hormone before its own thyroid becomes functional. Thyroid hormone requirements also increase during pregnancy. Women with established hypothyroidism may require medication adjustments and closer monitoring before and during pregnancy. Untreated overt thyroid dysfunction is associated with adverse pregnancy outcomes, making appropriate medical management important. But here’s what I want women to understand. Not every irregular cycle is caused by thyroid dysfunction. Not every woman struggling to conceive has a thyroid problem. And correcting thyroid function won’t necessarily resolve every fertility concern. The goal isn’t to chase an arbitrary ‘optimal’ TSH number. It’s to identify clinically meaningful thyroid dysfunction and understand how it fits into the wider fertility picture. At Nourishing Apothecary, our fertility naturopaths Mel and Chloe support women and couples with fertility concerns, including pathology interpretation, preconception nutrition and individualised care alongside appropriate medical management. Consultations are available Australia wide via Zoom.
