You may have heard of a bullhorn lip lift, a surgery that shortens the distance between the nose and upper lip. And yes — shortening this area can improve facial proportions and make the face look more youthful. But this distance doesn’t change on its own. As the facial structure changes with age, multiple proportions change together: the midface can become longer and flatter, the upper jaw can shift downward and backward, and the cheekbones, lips, and lower face change with it. A bullhorn lip lift changes one distance, but it doesn’t restore the rest of the facial structure. That’s why simply shortening the philtrum doesn’t necessarily make the whole face look significantly younger. If you want to work on the face as a whole, you need to look at the underlying structure — the position of the facial bones, posture, muscles, fascia, and the tension patterns that influence them.
