Madara was playing with him, because when Guy got that good hit in. He was surprised and laughed saying he almost died. It's like boxing a teenager and being surprised when they get a clean hit on your face. When Madara was fighting Naruto and Sasuke, he was not playing, he was dead serious on killing them. No thrill of the battle, pure survival.
